Instructions for compensating stock value losses

How to report a stock value loss:

• Download the Stock Value Loss Form and send the completed form by email to
• CustomerService.CH@zentiva.ch by the end of the month in which the price change occurs.

If you are unable to download the form, please contact our Customer Service team by email (CustomerService.CH@zentiva.ch) or reach out to your local sales representative. They will be happy to provide the form for you.
